Tactical error – don’t look at your gun when reloading. Keep your eyes on the threat. Can’t tell from the video, but he may have exposed too much of his body by leaning around that barrel (and IMHO) as well as being too close. You always want some distance from cover to lessen the chance of being hit by fragments from a near miss.
